Plastic surgery in New York City costs more than almost anywhere in the country, and the spread inside the metro is larger than the spread between regions. Across 25 surgical procedures, Manhattan all-in prices run $5,000–$30,000. New Jersey prices are 10–25% lower for the same credentials. This index prices all 34 side by side so you can see which tier you are actually comparing.
Methodology, and why the gap exists
Every figure in this index is all-in: the surgeon's fee plus anaesthesia plus the facility. Headline "from" prices almost always quote the surgeon fee alone and then add facility and anaesthesia later, which is why two quotes for the same operation can differ by $5,000 without either practice being dishonest.
Three things drive the Manhattan premium, and none of them is skill:
- Facility and operating-room rates. CMS Medicare data for New York hospitals shows average billed charges ranging from roughly $13,000 at New York Eye and Ear Infirmary to over $127,000 at NYU Langone, with Mount Sinai near $91,000 and Lenox Hill near $38,500. Billed charges are list prices, not what anyone pays — but the spread between them is exactly the signal here, and the anaesthesia and facility line on a Manhattan quote is where much of it lands.
- Overhead. Park Avenue and Madison Avenue rent, and Manhattan clinical staff salaries, pass straight into the quote. Independent practice fee data puts Manhattan physician costs roughly 17% above the national baseline, with individual procedures varying 3–5x even within the city.
- Reputation premium. Surgeons with national profiles price on demand rather than on cost. Some of that is genuinely earned experience; some of it is geographic expectation. Both are real, and neither is disclosed in a quote.
What this means practically: trade geographic premium for geographic value, never for geographic safety. Moving a $12,000 Manhattan operation to an accredited New Jersey facility can save $2,000–$3,000 with an identical credential. Moving it to an unaccredited suite to save $4,000 is a bad trade at any price.

Across our 34 procedures, surgical all-in costs in the five boroughs span $5,000–$30,000: breast augmentation $7,000–$14,000, rhinoplasty $7,000–$13,000, tummy tuck $12,000–$18,000, facelift $15,000–$30,000, BBL $10,000–$22,000 and liposuction $4,000–$16,000. Injectables start around $300 per unit.
Facility rates, overhead and a reputation premium, in that order. CMS data shows average billed charges across New York hospitals ranging from about $13,000 to over $127,000, while Medicare payments for the same facilities run $2,400 to $23,500. The spread between billed and paid is the market signal, and on a cosmetic quote it lands in the facility and anaesthesia lines.
Consistently 10–25% cheaper, because facility, staffing and rent costs are lower. Critically, many New Jersey surgeons hold Manhattan hospital privileges and consult in Midtown, so a cheaper New Jersey price no longer means a weaker credential. Verify ABPS status and you lose nothing but $2,000–$3,000.
